2026-27 UIL PHYSICAL/MEDICAL HISTORY FORMS - Please click the link for the 2026-27 UIL Physical/Medical History Forms. For 7th and 8th graders, these forms must be completed before the first day of school. The forms cannot be dated before May 1, 2026. 2026-27 RANK ONE FORMS - The Rank One Forms must be completed for all incoming 7th & 8th grade athletes before the first day of school. To complete the forms, the parents must have their child's student id number. Please email Coach Graham, an Assistant Athletic Trainer - grahamj@hpisd.org, if you have any questions.
