2026-27 RANK ONE FORMS - The Rank One Forms must be completed for all 7th & 8th grade athletes before the first day of school. To complete the forms, the parents must have their child's student id number. Please email Coach Graham, an Assistant Athletic Trainer - grahamj@hpisd.org, if you have any questions.  Link

2026-27 ATHLETIC PHYSICALS - Saturday, May 16th @ the Weber Indoor Facility. 7th & 8th grade - 8:00 a.m.-11:00 a.m., and 9th-12 grade 1:00 p.m.-4:00 p.m. Please bring the physical form and completed medical history form on the 16th. Please make sure a parent and the athlete has signed the medical history form before arriving. The physician onsite will complete the physical form. Cost - $25 - cash or check only (no Apple Pay, Venmo, etc). Make checks to Highland Park Sports Medicine. Physicals cannot be dated before May 1, 2026.
