5/26/2026	2026-27 UIL PHYSICAL/MEDICAL HISTORY FORMS - Please click the link for the 2026-27 UIL Physical/Medical History Forms. For 7th and 8th graders, these forms must be completed before the first day of school. The forms cannot be dated before May 1, 2026. For all 7th and 8th graders, please email the forms as a pdf to the Assistant Athletic Trainer, Jordan Graham, grahamj@hpisd.org. No photos are allowed. Go Scots! Link
5/3/2026	2026-27 RANK ONE FORMS - The Rank One Forms must be completed for all incoming 7th & 8th grade athletes before the first day of school.
